Adult Social Care - Services aim to deliver high quality services to promote the health, well-being and independence of those Borough of Poole residents most in need.

We do this by

Who we help

We do this by:

  • promoting independence and enabling people to live as independently as possible in their own homes

  • supporting carers

  • protecting the most vulnerable people, who often need to be looked after in residential or nursing homes, and those being cared for at home 

  • enabling people to be discharged from hospital as speedily and safely as possible.

  • providing day services which in addition to providing care give access to a wide range of opportunities within the community

  • providing short-term domiciliary care services for people  recently discharged from hospital

  • providing support to people with drug and/or alcohol dependency issues

  • promoting the well-being of vulnerable people within the wider community through a range of partnerships with the NHS locally and a range of voluntary organisations

  • providing care management, assessment and some direct provision for those assessed through the Fair Access to Care Eligiblity Criteria

  • promoting the well-being of vulnerable people who often need to be looked after in residential or nursing homes, and those being cared for at home.

  • people who look after a relative or friend - carers

  • people with dementia

  • people with a physical disability

  • people with a learning disability

  • people with a mental health issue

  • people with sight or hearing impairment

  • people who misuse drugs and alcohol

  • people who are leaving hospital

Everyone has the right to ask us for help and we will endeavour to assist, either by meeting your needs ourselves or by directing you to someone else who can.

The Care Quality Commission (CQC) inspected the Poole Addictions Community Team (PACT) and Gateway (the improving Access to Psychological Therapies Services). The CQC Inspection Report 2013 has been published.

The Big Plan explains how Bournemouth and Poole will work together to provide health and social care services for adults with learning disabilities and is now out for public consultation.
