Poole Intermediate Care Service (PICS)
Poole Intermediate Care Service (PICS) is a new joint service for adults and older people in the Borough of Poole designed to:
- prevent unnecessary admission to hospital/residential care
- facilitate rapid discharge from hospital
- support carers at times of crisis.
This service may include Senior Nursing and Social Work staff undertaking assessments, providing treatment and devising ongoing care packages; with front line practical support provided by a team of Intermediate Care Assistants. In practice an older person might receive support to dress, wash, cook, take medication and/or mobilise, in addition to nursing tasks appropriate to their condition. It is funded by the Borough of Poole and NHS Bournemouth and Poole. ![]()
Referral
Referral to the Service can be made via the Adult Social Services Helpdesk on 01202 633902, email sshelpdesk@poole.gov.uk, although it is more commonly as a consequence of the team's relationship with hospital and GP surgery staff. There is no dedicated referral form.
