Supporting Information Forms for the VA and VC schools
If you are intending to apply for a place at a Voluntary Aided or a Voluntary Controlled school on religious grounds you must submit a "supplementary information form" which should be signed by a Priest/Vicar/Minister of Religion. There is not a common information form for all schools, there are separate forms for individual schools. Please select the appropriate form(s) for the school(s) to which you will be applying for a place on the right hand side of this page.
The forms, when completed must be returned by the published closing date, either:
- if you are filling out a paper application form it should be returned with that form, or
- if you are completing an on-line application the form it should be returned to The School Admissions Team, Children & Young People's Integrated Services, The Dolphin Centre, Poole, BH15 1SA
If you do not return a completed supplementary information form by the published closing date your application will still be considered but will not be considered under the religious criterion.
